Location 34 Ledgebrook Dr, Mansfield (10179) Overview Hartford HealthCare at Home, the largest provider of home‑care services in Connecticut, invites you to become part of its comprehensive network as an Occupational Therapist. The role supports a person‑centered model of care, providing integrated services in a variety of settings—including inpatient, outpatient, home care, skilled nursing, and assisted living—while fostering professional growth. Responsibilities Perform independent evaluations of patients’ functional abilities, assessing upper extremity strength, motor control, joint mobility, pain, balance, coordination, and daily living activities. Develop individualized, patient‑centered treatment plans, establish measurable goals in collaboration with patients and families, and continuously evaluate and revise plans based on progress. Deliver therapeutic techniques tailored to patients’ medical status, supporting fine and gross motor skills, leisure/recreation, and productive activities. Delegate patient‑care responsibilities to Certified Occupational Therapy Assistants and provide clinical supervision to students and rehabilitation aides. Document all services accurately and timely, including evaluations, care plans, goals, progress notes, discharge summaries, and transition plans, in compliance with coding standards and licensure requirements. Coordinate patient transition and discharge planning, recommending community resources and collaborating with nurses, social workers, and other health‑care team members. Maintain a clean, infection‑controlled work environment and restock supplies as necessary. Participate in continuing education to uphold clinical proficiency and meet Connecticut licensure obligations. Qualifications Required: Bachelor of Science in Occupational Therapy. Preferred: Master’s or Doctorate in Occupational Therapy. Experience Preferred: Prior experience as an Occupational Therapist. Perform other related duties as required. Licensure, Certification, Registration Current State of Connecticut Occupational Therapy license required.
