We are currently seeking a full-time Registered Nurse Case Manager who is passionate about providing compassionate, individualized care to patients and families during their most vulnerable moments.

What You’ll Do:

• Manage a caseload of hospice patients, providing skilled nursing visits and coordinating all aspects of care.
• Develop and implement individualized care plans in collaboration with the interdisciplinary team.
• Educate patients and families on hospice philosophy, care expectations, and symptom management.
• Act as the primary clinical contact for assigned patients, ensuring comfort, dignity, and quality of life.
• Maintain accurate and timely documentation that meets all regulatory and organizational standards.
• Participate in IDG meetings and collaborate with aides, chaplains, social workers, and medical directors.

What We’re Looking For:

• Current Texas RN license in good standing.
• Minimum 1 year of nursing experience (Hospice, Home Health, or ICU/ER preferred).
• Strong clinical judgment and communication skills.
• Empathy, professionalism, and a heart for hospice care.
• Valid driver’s license and reliable transportation.
