Nurse Practitioner Carease Health - PA is seeking a dedicated and compassionate Nurse Practitioner to join our team of healthcare professionals in Butler County, PA. As a Nurse Practitioner , you will play a vital role in delivering high-quality preventative care to residents in skilled nursing facilities, working collaboratively with our team to reduce hospitalizations and improve patient outcomes. About the Role: As a hybrid role, you will split your time between on-site visits with patients and remote work from home, supported by a dedicated scribe team for efficient documentation. Your success in this role depends on building strong relationships with the facility, maintaining a patient-centered approach, and being a collaborative team player.
