The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities.Complete an initial, comprehensive and ongoing assessments of patient and family to determine hospice needsProvide a complete physical assessment and history of current and previous illness(es)Provide professional nursing care by utilizing all elements of nursing processAssess and evaluates patient's statusUse health assessment data to determine nursing diagnosisDevelop a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actionsInitiate appropriate preventive and rehabilitative nursing procedures.Administer medications and treatments as prescribed by the physician in the physician's plan of careCounsel the patient and family in meeting nursingProvide health care instructions to the patient per assessment and planAssist the patient with the activities of daily livingFacilitate the patient's efforts toward self-sufficiency and optional comfort careAct as Case Manager and assumes responsibility to coordinate patient care for assigned caseloadJob RequirementsActive CA RN LicenseHospice experience preferredAbility for prolonged or considerable walking or standingAbility to lift, position, and/or transfer patientsAbility to lift supplies and equipment.Ability for considerable reaching, stooping, bending, kneeling, and/or crouching.Visual acuity and hearing to perform required nursing skillsMaintains a current CPR certificationWhy First Call?First Call Hospice is part of the Cornerstone Group with close to 80 agencies throughout the country.
