Therefore, final offer amounts may vary from the amount stated.Job Description & Essential FunctionsProvide a safe, welcoming, and clean environment for our clients and familiesPerform trauma-informed assessments to identify the unique needs of clientsConduct short-term individual, couples/family, and group therapy for individuals and families with mental health and substance use disorders across the lifespanMaintain a yearly production expectation based on hours workedComplete billable services within 24-hour of eventUtilize collaborative documentation as a treatment modalityEngage in effective coordination of care across health and behavioral health services to facilitate wellness and recovery of the whole personMake referrals or link to care coordination, peer support and recovery services when neededSupport transitions of care, including assisting with the development of aftercare planning with clients and familiesProvide crisis management services and coordinate with Emergency Services as appropriateServe as a community educator by providing mental health information to relevant individuals, organizations, and groupsUtilize evidence-based interventions, including motivational interviewing, to develop person-centered treatment plans through collaborative documentationMaintain a flexible schedule that is conducive to providing services in an open access model that will include in-person and telehealth servicesMaintain a caseload of clients and be open to triage and accommodate walk-in appointmentsMaintain record-keeping as required by CFS, funding sources, and accreditation authoritiesParticipate in all supervision and training activities as required by CFS, funding sources, and accreditation authoritiesKeep EHR calendar up to date to support centralized schedulingMaintain CANS certificationIf licensed, complete continued education to maintain clinical professional licensure and ensure that licensure remains activeReasonable accommodations may be made to enable individuals with disabilities to perform these essential functions.Other DutiesPlease note this job description is not designed to cover or contain a comprehensive listing of activities, duties or responsibilities that are required of the employee for this job.
