Job Title: Nurse III – Registered Nurse Case Manager Location: 3401 N University Ave, Lubbock, TX (Lubbock State Supported Living Center) Salary: $7,532.75 – $8,886.16 Monthly Shift: Day (First) Employment Type: Full time, Regular, Exempt Telework: Not Eligible Travel: Up to 5% Responsibilities Provide complex nursing care and assessment for residents at the Lubbock State Supported Living Center. Conduct physical assessments (monthly, quarterly, annually, as needed) and develop integrated care plans. Document health status, review medication administration records, and report trends to appropriate staff. Collaborate with the interdisciplinary team (IDT) to ensure coordinated, high‑quality care. Participate in quality assurance reviews, training, committees, and coaching of new RN case managers. Attend on‑campus medical appointments and coordinate off‑campus specialist visits when required. Maintain competence in nursing best practices, safety, and infection‑control protocols. Prepare nursing care plans that accurately direct the implementation of all necessary care. Mentor and coach junior staff and new RN case managers as assigned. Essential Job Functions (EJFs) Attend work regularly and maintain shift schedule, including overtime and nights as required. Serve in on‑call status on a rotating schedule to meet operational needs. Conduct rounds on assigned homes, observe interactions, and provide proactive nursing care. Engage in proactive problem solving and consensus building within the IDT. Update patient information in automated systems and coordinate communication across disciplines. Review physician orders, medications, and incident reports to identify trends and take appropriate action. Participate in competency training and continuing education to maintain licensure. Maintain professional relationships with residents, families, staff, and visitors. Assist in disaster response and continuity of operations as required. Knowledge, Skills and Abilities Proficient in nursing techniques, standards, and procedures; knowledge of health‑care laws and regulations. Understanding of ICF‑IID rules, policy, and procedures related to State Supported Living Centers. Ability to interact appropriately with individuals with intellectual/developmental disabilities and their families. Skill in time management, prioritization, and independent judgment. Excellent verbal and written communication skills. Proficient with electronic health record systems. Maintains current CPR certification and emergency response knowledge. Analytical ability to integrate information and develop patient‑centered care plans. Qualifications Registered Nurse license in Texas or a state that recognizes reciprocity. At least 1 year of experience as a practicing RN. Experience in assessment and documentation of health status required. Preferred: 2+ years in a health agency/hospital; Bachelor’s Degree. Preferred: Experience working with individuals with intellectual or developmental disabilities. Additional Information Applicants must pass background checks, drug screening, and appropriate registry checks. The position may require overtime, extended hours, weekends, and holidays.
