Geriatrics Nurse Practitioner

If you re an experienced Nurse Practitioner with geriatric care experience, this role focuses on managing chronic conditions and helping seniors remain safely in their homes, maintaining their independence.

What You ll Do

• Conduct biannual health assessments, develop care plans and physical exams
• Manage chronic conditions for geriatric participants
• Develop and adjust treatment plans
• Be part of a tight knit interdisciplinary team
• Educate participants and family members

What You ll Need

• Licensed Nurse Practitioner in Ohio
• CPR and First Aid certification
• 4+ years as a Nurse Practitioner seeing adult patients
• Valid driver s license and local travel ability
• Mission-driven and passionate for the elderly

This pay range represents Covelo Direct's good faith minimum and maximum range for this role at the time of posting.
