Travel Physical Therapist (PT) – Home Health | Ohio
We are currently seeking a compassionate and motivated Physical Therapist for a 13-week Home Health travel assignment in Ohio. This is an excellent opportunity for therapists who enjoy working independently and helping patients improve mobility and quality of life in their home environment.
Position Details:
- Specialty: Physical Therapy
- Setting: Home Health
- Shift: Days
- Schedule: 8:00 AM – 5:00 PM
- Duration: 13 Weeks
- Guaranteed Hours: 40 Hours per Week
- Start Date: ASAP
Responsibilities:
- Evaluate and develop individualized treatment plans
- Provide therapeutic exercises and interventions to improve patient function and mobility
- Educate patients and caregivers on treatment plans and home exercise programs
- Monitor patient progress and modify treatment plans as needed
- Maintain accurate and timely documentation
- Collaborate with interdisciplinary healthcare teams to ensure optimal patient outcomes

Requirements
- Active Physical Therapist License
- Minimum 1 Year of Physical Therapy Experience
- Current BLS Certification
- Home Health experience preferred
- Strong communication and patient education skills
- Ability to work independently and manage a flexible schedule
- Reliable transportation for patient visits
