This role works directly with patients, caregivers, and a multidisciplinary clinical team to ensure safe, supported transitions of care, reduce avoidable hospitalizations, and improve long-term disease outcomes.**Responsibilities*** Participates in the design, conduct, and evaluation of implementation research and cohort studies focusing on transitional care interventions and their associated health outcomes* Coordinates follow-up appointments with primary care and subspecialty teams, including cardiology, nephrology, and pulmonology* Follows an assigned cohort and outreaches patients telephonically to ensure effectiveness of interventions* Collaborates with TCE clinical team to support implementation of care plans and therapies* Identifies and escalates clinical and social risks, including worsening symptoms, medication access issues, and unmet social needs, to the TCE clinical team* Assists with implementation of departmental transitional care protocols and pathways* Supports patients in navigating the healthcare system, including use of patient portals, telemedicine visits, and remote monitoring tools* Collects qualitative and quantitative data from multiple sources including patients, caregivers, clinicians, and electronic health records* Analyzes and interprets data in Excel, Power BI or other software**Requirements*** Bachelor’s degree required, life sciences or public health background preferred* MS Office proficient, Power BI or Tableau experience preferred* Ability to manage multiple tasks and priorities with exceptional attention to detail* Bilingual/Spanish-speaking preferred* Evidence of excellent interpersonal and communication skills, creative problem solving, critical thinking and leadership* Previously demonstrated ability to facilitate group processes and work cohesively and collaboratively as member of the interdisciplinary team* Qualified candidates must be able to effectively communicate with all levels of the organizationMontefiore Medical Center is an equal employment opportunity employer.
