Work Environment

This position is based onsite, providing home care services directly to patients in their residences. The facility's layout encourages a warm and inviting atmosphere that nurtures recovery while delivering exceptional care. You will work a standard schedule from Monday to Friday, 9 AM to 5 PM, with a patient caseload of one individual per hour, giving you the necessary time and focus to make a significant impact in your patients' lives.

Key Responsibilities

In this role, you will:

• Conduct comprehensive assessments of each patient's physical condition to create personalized treatment plans.
• Develop specific treatment goals with measurable outcomes that align with each patient's needs.
• Implement therapeutic interventions during each session that supports long-term recovery, including exercises and manual therapy techniques.
• Educate and empower patients and their families on appropriate home care techniques to ensure continuity of care beyond therapy sessions.
• Collaborate closely with multidisciplinary team members, including other therapists and healthcare professionals, to ensure cohesive and integrated patient care.
• Maintain accurate and timely documentation of patient progress using Electronic Medical Records (EMR) systems, ensuring compliance with regulatory standards.

Patient Population / Client Focus

Your focus will be on a diverse patient population, including individuals recovering from orthopedic injuries, surgeries, chronic pain conditions, and neurological disorders. You'll engage with patients at various stages of recovery, tailoring your approach to each unique situation. The trust and connection you build with patients will be essential in facilitating their recovery and achieving optimal outcomes.

Qualifications

The ideal candidate will possess:

• A valid Physical Therapist license in the state of Michigan.
• A Bachelor's Degree in Physical Therapy; however, candidates with a Master's or Doctorate in Physical Therapy (MS or DPT) are encouraged to apply.

Preferred Experience

While we welcome new graduates, candidates with at least two years of experience in physical therapy are preferred, particularly those with knowledge of manual therapy techniques. Compensation and Benefits
