Certs & Licensure: Current license in the state of Arizona- Required BLS (American Heart Association) - Required *Please note that traveler must have an AZ license in hand*

In order to obtain an AZ OT License, the candidate must submit a fingerprint clearance card along with the application. Fingerprint clearance estimated turnaround time is 4-6 weeks. OT Application estimated turnaround time is 3-6 weeks- the whole process can take anywhere from 7-12 weeks.

Special Requirements: The department is open 7 days a week. A weekend day will be required. Preferred generalist who is confident with medical diagnosis. Preferred ability to manage mod- max A patients, co-treatments, ability to work with ventilated patients, ability to pivot easily and pick up additional evaluations throughout the day.

Guaranteed Hours: 40 hours/week, facility is allowed to call off 6x throughout a 13-week assignment
