Job Summary:

We are seeking a compassionate and dedicated On-Call Caregiver to join our team at WI08, SYNERGY HomeCare of Madison in Monona, Wisconsin. As an On-Call Caregiver, you will provide personalized care and support to our clients in the comfort of their own homes. Responsibilities:

• Provide one-on-one care and companionship to clients in their homes, focusing on their unique needs and abilities

• Assist with daily living activities, including personal care, meal preparation, and light housekeeping

• Encourage and support clients in their daily routines, promoting independence and autonomy

• Maintain accurate and detailed records of client care, including progress notes and treatment plans

• Collaborate with other caregivers and healthcare professionals to ensure seamless care coordination

• Demonstrate a commitment to excellence, prioritizing client satisfaction and well-being above all else

Requirements:

• High school diploma or equivalent required

• Prior experience in the healthcare industry, preferably in a caregiving role

• Ability to lift up to 25 pounds and perform physical demands of the job

• Valid driver's license and reliable transportation
