About the Role:
Join Integrated Community Living and Par in Allentown, PA, as a Life Sharing and In-home Support Coordinator, where you can make a meaningful difference in the lives of individuals with disabilities. The coordinator will head and manage the department of Lifesharing and In-Home Support. This role balances communitybased work and administrative responsibilities. This position ensures that services are delivered safely, compliantly, and in alignment with state and county regulations, while promoting dignity, independence, and meaningful community connection.

Responsibilities:
  • Support and Guidance: provide ongoing support to the individual and provider, helping to navigate any challenges that arise and ensuring that the individuals needs are met according to their person-directed support plan.
  • Facilitating Life Sharing: help match individuals with disabilities to families or individuals who can provide a supportive living environment. This involves assessing the needs of both the individual and potential provider to ensure a good fit.
  • Training and Resources: Responsible for training Life Sharing providers, offering resources, and ensuring that they understand their roles and responsibilities in supporting the individual.
  • Monitoring and Evaluation: Regularly monitoring the Life Sharing arrangements to ensure that the individual is thriving and that the provider is fulfilling their commitments.
  • Develop and implement individualized care plans tailored to client needs.
  • Maintain accurate documentation of client progress and support activities.
  • Collaborate with families and interdisciplinary teams to ensure holistic care.
  • Provide transportation for clients to appointments and community events.
  • Participate in ongoing training and professional development opportunities.
Requirements:
  • Bachelors degree plus 2 years of experience supporting individuals with intellectual and developmental disabilities (IDD), or
  • Associate degree/60 credits plus 4 years of IDD experience, or
  • High school diploma plus 6 years of IDD experience
  • Valid drivers license and reliable transportation.
  • Strong communication and interpersonal skills.
  • Compassionate and patient demeanor with a commitment to client advocacy.
  • Experience in caregiving or support roles is a plus.
  • First Aid and CPR certification preferred.
  • Background check clearance required.
