Case Management Coordinator - Per Diem - Days - 8hr QVH

Job Summary Under the direction of the Case Manager or Discharge Planner, the Case Management Coordinator assists with the clerical needs of discharge planning. Responsibilities Fax inquiries and other communications. Arrange ambulance transportation. Make phone calls to skilled nursing facilities, home health agencies, durable medical equipment companies, infusion agencies, and family members. Deliver important Medicare messages to patients. Document all activities appropriately in Meditech. Check department voicemail and retrieve orders from the printer, distributing to appropriate staff. Retrieve faxes and document in BAR. Fax CCS referrals and Interqual reviews, documenting in BAR. Order supplies, schedule meetings, and greet visitors. Job Requirements Minimum Education Requirement High School diploma or equivalent preferred. Minimum Experience Requirement One year of general office/clerical experience preferred. Proficient in Microsoft Office. Knowledge of medical terminology preferred. Excellent customer service and organizational skills required. Pay Range: $30.00 - $30.00 #J-18808-Ljbffr Emanate Health
