Reports To: Intimate Health Program Administrator / Program Leadership
Location: South Carolina – Clinic & Community-Based Outreach
Schedule: Full-Time or Part-Time, based on program needs

Position Summary

The Intimate Community Healthcare Worker / Screener serves as a frontline member of JHN’s Intimate Health and community outreach programs. This position provides health education, community outreach, health screenings, patient navigation, and linkage to healthcare services.

The Community Healthcare Worker/Screener works both inside the JHN clinic and throughout the community, including rural and underserved areas. The position helps individuals access confidential, respectful, and stigma-free sexual and intimate healthcare services.

The position is designed for someone who enjoys working directly with people, building community relationships, and helping individuals overcome barriers to healthcare.

Key Responsibilities

• Provide community-based and clinic-based health screenings in accordance with JHN policies, training, standing procedures, and applicable requirements.
• Conduct approved rapid HIV and Hepatitis C screenings after completing required training and certification.
• Assist with STI screening activities, including patient education and appropriate specimen collection processes within the employee's training and scope.
• Educate individuals about HIV, STIs, Hepatitis C, PrEP, Doxy-PEP, prevention, treatment, and other intimate health services.
• Provide confidential, nonjudgmental education about sexual and intimate health.
• Explain available JHN services and help individuals understand their healthcare options.
• Conduct community outreach at health fairs, churches, pharmacies, community centers, colleges, recovery organizations, shelters, rural communities, and other community locations.
• Assist with JHN mobile health, pop-up clinic, and rural outreach activities.
• Help identify individuals who may benefit from PrEP, STI services, primary care, Hepatitis C treatment, or other healthcare services.
• Connect patients with JHN clinical staff and appropriate community healthcare providers.
• Assist individuals with scheduling appointments and completing referrals.
• Follow up with individuals when appropriate to support successful linkage to care.
• Help address barriers such as transportation, insurance, medication access, appointment scheduling, and understanding the healthcare system.
• Provide information regarding community resources and supportive services.
• Maintain accurate documentation of screenings, outreach activities, referrals, and services provided.
• Enter required program information into approved JHN systems.
• Maintain testing supplies and assist with inventory management.
• Prepare screening and outreach materials before community events.
• Assist with setup and breakdown of community screening events and mobile outreach activities.
• Follow all infection-control, testing, specimen-handling, and safety procedures.
• Protect patient confidentiality and comply with HIPAA and JHN privacy policies at all times .
• Participate in required staff meetings, trainings, certifications, and continuing education.
• Represent JHN professionally within the community.
• Perform other community health and program duties as assigned.

Community Health Worker Responsibilities

The employee will serve as a connection between JHN and the communities we serve by helping individuals navigate healthcare and social-service systems.

• Follow-up after screenings
• Health education
• Community resource referrals
• Transportation coordination
• Insurance and healthcare navigation assistance
• Medication-access referrals
• Community outreach
• Identifying barriers to care
• Supporting patients in remaining engaged in healthcare

Intimate Health Screening

Depending on training, certification, program requirements, and applicable scope, screening responsibilities may include:

• Rapid HIV screening
• Rapid Hepatitis C screening
• STI screening support
• Specimen collection support
• Patient intake and risk assessment
• Prevention education
• PrEP education and referrals
• Doxy-PEP education and referrals
• Linkage to HIV care
• Linkage to Hepatitis C treatment
• STI treatment referrals
• Chronic disease screening support during community events

Requirements:

All screenings must be performed according to JHN policies, manufacturer instructions, applicable standing procedures, and required state or program standards.

Qualifications

• High school diploma or GED required.
• Community Health Worker experience or certification preferred but not required.
• Previous experience in healthcare, public health, community outreach, HIV/STI prevention, patient navigation, or social services is preferred.
• Willingness to complete required HIV, STI, Hepatitis C, testing, counseling, and Community Health Worker training.
• Strong communication and interpersonal skills.
• Ability to communicate with individuals from diverse backgrounds without judgment.
• Comfortable discussing sexual and intimate health topics professionally.
• Ability to maintain strict patient confidentiality.
• Basic computer and documentation skills.
• Ability to work independently and as part of a team.
• Reliable transportation and ability to travel to community outreach locations as required.
• Some evening and weekend availability may be required for community events.

Success in this position includes maintaining confidentiality, completing accurate documentation, conducting quality screenings, participating in community outreach, successfully connecting individuals to healthcare, maintaining required certifications, and representing JHN professionally.The employee will also be expected to contribute toward JHN's program goals related to the number of individuals reached, screened, referred, and successfully linked to appropriate healthcare services.
