• Provide physical therapy services under the supervision of a Physical Therapist
• Assist with treatment plans to improve patient mobility and function
• Deliver care to patients in a home health setting
• Monitor patient progress and report updates to the supervising therapist
• Educate patients and caregivers on exercises and therapy techniques
• Travel between patient homes within the assigned service area
• Maintain accurate documentation and follow established care plans

Charting system: Homecare Homebase (experience preferred) Active Massachusetts PTA license required Home health experience required Current BLS certification required
