About the Role The Clinical Health Advocate (Licensed Practical Nurse) is an integral part of our care team, owning patient relationships and backing a collaborative care plan that drives meaningful impact in our patients’ lives. We’re looking for a teammate who is an innate people person, loves helping others, and has a knack for tenacious problem solving. This is a part-time (up to 24 hours per week) hybrid role. You'll spend time in the field, traveling to see patients within Aroostook County or working from an office in Presque Isle, with some remote work from home options. The role is 50% clinical and 50% administrative, scheduled Monday through Friday from 8 a.m. to 5 p.m. ET. Key Responsibilities Clinical At patient visits, obtain and document vital signs (blood pressure, temperature, pulse, respirations, weight, height) in the EHR. Perform venipuncture and capillary blood collection for laboratory testing following proper phlebotomy protocols. Collect, process, and prepare any specimens for laboratory analysis. Administer immunizations and injections (as able) per physician orders while maintaining accurate vaccine records. Assist providers with minor procedures and examinations as needed. When in the field, relay pertinent environmental advantages or disadvantages that will impact the patient’s well‑being to the care team; complete physical and social screenings as assigned by the provider. Educate patients on treatment plans, medications, and follow‑up care as directed by providers. Document patient interactions, clinical data, and care activities accurately in the electronic health record. Carry out population health care gap closures efforts and sprints per payer contract. Ensure proper sterilization and maintenance of medical equipment. Follow infection control protocols and maintain compliance with safety standards. Be available for field and urgent deployment within your market region even on unplanned field days. Connect patients with Galileo services and teams, including clinical and administrative functions. Admin Serve as the local “face” of Galileo, representing the organization professionally, embodying our mission, and building meaningful relationships with stakeholders. Own overall empaneled patient chart management, preparing charts medically and demographically to ensure readiness and smooth care delivery. Run the list in partnership with the care team to ensure orders, care coordination, and constant communication is seamless for each patient. Maintain exam room, mobile unit, and/or office cleanliness, organization, and adequate supply levels. Manage inventory of clinical supplies and medications, placing orders as needed. Route inbound mail appropriately and send patient letters or medical equipment as needed. Coordinate with insurances to submit and complete prior authorizations and referrals. Request, follow up, and file medical records with local hospital systems, PCPs, and area HIES. Submit population health documents to maximize compliance with payer contracts as needed. Communicate with patients via text, video, or phone as needed for care coordination, scheduling, and reactive needs. Participate in projects and events as assigned by the Director of Operations. Qualifications At least 2 years of clinical experience as a Licensed Practical Nurse. Experience with phlebotomy is a must. Experience with injections (vaccines) is preferred. Reliable access to personal transportation. Strong people skills and the ability to connect with anyone. Creative problem‑solving skills that can be leveraged to empower others and impeccable organizational skills that foster reliability and trustworthiness.
