Duties
Major duties include but are not limited to:
- Responsible for completing psychosocial history and assessments, intakes, advance directives, treatment planning, case management, individual, couples and group counseling and coordination of discharge plans for veterans admitted to the Substance Use Disorder (SUD) Residential Rehabilitation Treatment Program (RRTP).
- Functions as a member of the interdisciplinary treatment team and is responsible for providing biopsychosocial assessments, treatment planning, case management, individual, family, and group counseling, crisis intervention, discharge planning, and coordination of care.
- Works with a wide range of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds who are experiencing a variety of psychiatric, medical, and social problems.
- Knowledgeable of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.
- Provide comprehensive clinical social work services to Veterans with serious mental illness, co-occurring substance use disorders, and complex psychosocial needs requiring residential psychiatric rehabilitation.
- Services are provided utilizing recovery-oriented, trauma-informed, and Veteran-centered principles designed to promote psychiatric stability, improve psychosocial functioning, foster community reintegration, and facilitate successful transition to outpatient care.
- Provides ongoing case management services including coordination of psychiatric, medical, vocational, housing, financial, legal, and community resources throughout the Veteran's residential admission.
- Assists Veterans in developing independent living skills, improving psychosocial functioning, strengthening community integration, and achieving individualized recovery goals through rehabilitation-focused interventions.
- Coordinates discharge planning to promote continuity of care through timely referrals to outpatient mental health services, primary care, vocational rehabilitation, homeless programs, community providers, and other appropriate VA and community resources.
- Collaborates with psychiatrists, psychologists, nurses, peer support specialists, recreation therapists, health technicians, Whole Health staff, and other interdisciplinary team members to ensure coordinated Veteran-centered care.
- Provides education to Veterans and family members regarding recovery, treatment recommendations, VA benefits, advance directives, wellness strategies, and available community resources.
- Documents assessments, treatment plans, clinical interventions, case management activities, discharge planning, and other clinical services in the electronic health record in accordance with VA policy and professional practice standards.
- Maintains professional competency through participation in mandatory education, continuing professional development, and required clinical training.

Work Schedule: Full-Time, 8:00am-4:30pm

Pay: Competitive salary and regular salary increases. When setting pay, a higher step rate of the appropriate grade may be determined after consideration of higher or unique qualifications or special needs of the VA (Above Minimum Rate of the Grade).
Paid Time Off: 37-50 days of annual paid time offer per year (13-26 days of annual leave, 13 days of sick leave, 11 paid Federal holidays per year). Selected applicants may qualify for credit toward annual leave accrual, based on prior [work experience] or military service experience.
Parental Leave: After 12 months of employment, up to 12 weeks of paid parental leave in connection with the birth, adoption, or foster care placement of a child.
Child Care Subsidy: After 60 days of employment, full time employees with a total family income below $144,000 may be eligible for a childcare subsidy up to 25% of total eligible childcare costs for eligible children up to the monthly maximum of $416.66
Retirement: Traditional federal pension (5 years vesting) and federal 401K with up to 5% in contributions by VA
Insurance: Federal health/vision/dental/term life/long-term care (many federal insurance programs can be carried into retirement)
