Job Title : Director of Social Services and Admissions

Location: Cozad, NE 

Department: Administration 

Reports to: Administrator 

Position Type: Full-Time 

Job Summary: 

The Director of Social Services and Admissions at Emerald Nursing & Rehab Cozad is a key leadership role responsible for overseeing and managing all social service and admission functions within the facility. This role ensures that residents receive comprehensive, compassionate, and personalized support to enhance their quality of life and meet their emotional, social, and psychological needs. This role also coordinates referrals and admissions to the building. The Director will work closely with interdisciplinary teams, families, and community resources to advocate for and support residents. 

Work Environment:

Office setting within a skilled nursing facility. 

Responsibilities: 

• Plan, develop, organize, implement, evaluate, and direct the social service programs. • Provide information to residents/families as to Medicare/Medicaid, and other financial assistance programs available to the resident. 
• Assist the Quality Assessment & Assurance Committee in developing and implementing appropriate plans of action to correct identified deficiencies. 
• Assume the authority, responsibility, and accountability of directing the social service department. 

Qualifications: 

• Minimum of a high school diploma with the ability to obtain NHCA 36-hour Social Work Designee within 6 months of starting. 
• Will maintain required credits in accordance with licensure requirements as needed. • Must be a minimum of 18 years old. 

Skills: 

• Proven experience as director of social services preferred. 
• Experience with referral review and admissions responsibilities preferred. 
• In-depth knowledge of long-term care procedures and resident care. 
• Great knowledge of legal regulations and best practices in healthcare. 
• Excellent ability to lead and develop personnel. 
• Great communication and people skills.
