Schedule: Days, 8 hours per day, 40 hours per week

Assignment Details: Guaranteed Hours: 40 per week Contract Length: 5 weeks

Roles & Responsibilities:

• Provide excellent patient care by collaborating with multidisciplinary teams to develop and implement case management programs.
• Serve as a liaison between patients, families, and healthcare providers to optimize patient outcomes.
• Coordinate the continuum of care for a defined population of patients from pre-admission through post-discharge.
• Assist patients and families in navigating the hospitalization process efficiently.
• Act as a consultant for the healthcare team regarding specific patient needs.
