Clinical Pharmacist

Job Summary The Clinical Pharmacist is a licensed professional responsible for delivering safe and appropriate pharmacy services to Option Care Health patients. The role is an entry‑level position in an alternate setting, coordinating patient services with nursing, monitoring progress toward goals, and educating patients, caregivers, staff, and other professionals about medication use. Job Responsibilities Determine the suitability of individual patients for home care by collecting and organizing patient‑specific information, assessing medical conditions and medication therapy, and defining outcome goals. Design, recommend, monitor, and evaluate patient‑specific pharmacy care plans; make recommendations for modifications to prescribed medication therapy or monitoring parameters; identify monitoring parameters and complete nutritional assessments. Modify pharmacy care plans based on evaluation of monitoring data and patient information, utilizing pharmacokinetic principles and laboratory value assessment. Provide medication‑use education to staff, other professionals, patients, and caregivers using effective counseling techniques. Ensure continuity of pharmaceutical care across home and other patient‑care settings, preparing and dispensing medications with appropriate labeling and safety practices, and complying with state/federal laws and USP standards. Manage, maintain, and troubleshoot medication administration equipment; provide training and report incidents and equipment errors; complete QAR documents and monitor inventory disparities. Respond concisely and timely to drug information requests from providers and patients, formulating responses based on literature analysis and attending educational seminars to maintain CEU requirements. Manage investigational drug products in accordance with regulations and policies; utilize pharmacy support personnel, prioritize workload, and ensure accuracy of their work. Comply with accreditation, legal, regulatory, and safety requirements in all activities. Maintain confidentiality of patient and proprietary information, observing legal and ethical safeguarding guidelines. Supervisory Responsibilities No supervisory duties. Education and Experience Requirements Graduate of an accredited School of Pharmacy. Current, active pharmacy license granted by the State Board of Pharmacy. At least one year of experience in sterile IV admixture procedures, or knowledge of non‑sterile compounding, immunizations and medication therapy management, completion of an ASHP accredited Pharmacy Residency Program, completion of an Advanced Pharmacy Practice Experience associated with an accredited school, or equivalent experience approved by VP Pharmacy or Senior VP Clinical Services. Qualifications Experience providing customer service to internal and external customers, meeting quality service standards and evaluating customer satisfaction. Basic proficiency in Microsoft Word for document editing, formatting, and basic functions. Basic proficiency in Microsoft Excel for workbook management, formatting, and basic functions. Basic proficiency in Microsoft PowerPoint for slide navigation, editing, and printing. Travel Requirements Willing to travel up to 5 % of the time for business purposes within the state. Preferred Qualifications & Interests Completion of an ASHP accredited Pharmacy Residency Program or an Advanced Pharmacy Practice Experience associated with an accredited school of pharmacy. Experience in home care, hospital, or other clean‑room settings. Management Requirements At least one year of indirect management experience, including assistance in developing, training, and assigning work to other team members. May perform other duties as assigned. Compensation Salary will be determined based on education, experience, skills, and market data. Pay range: $97,113.10 – $161,862.67.
