Per Diem / PRN Home Health Physical Therapist - $40-60 per hour

Job Title Physical Therapist Home Health (Per Diem / PRN) Job Location Denton, Texas (and surrounding areas: Sanger, Aubrey) Job Description Jet Health, operating as Trio Home Health, is seeking a licensed Physical Therapist to provide home‑based therapy services, including assessment, treatment planning, implementation, and evaluation for patients enrolled in our home health program. This role offers opportunities for one‑on‑one patient care with autonomy, balanced work schedule, and competitive compensation. Responsibilities Evaluate patients' physical and functional abilities to develop individualized care plans. Plan, implement, and modify physical therapy treatment programs based on medical referrals and assessments. Provide hands‑on therapy, exercises, and techniques to improve mobility, relieve pain, and restore function. Educate patients and families on therapeutic exercises, proper body mechanics, and injury‑prevention techniques. Collaborate with interdisciplinary healthcare teams to ensure coordinated and holistic patient care. Maintain accurate and timely documentation of patient evaluations, progress notes, and treatment plans. Participate in ongoing professional development activities to stay abreast of current trends and best practices in physical therapy. Qualifications Graduate of an accredited School of Physical Therapy. Current state license as a Physical Therapist, or a multi‑state license issued by a state member of the National Physical Therapy Compact (PT Compact), if practicing in a state member of the PT Compact. Minimum of two years of experience in a clinical care setting, with experience in home health, geriatrics, or medical‑surgical preferred. Excellent interpersonal and communication skills. Strong nursing skills as defined by generally accepted standards of practice. Reliable transportation with valid and current auto liability insurance. Ability to work flexible hours and travel locally as needed. Ability to lift up to 50 lbs. and perform physical tasks associated with patient care.
