Responsibilities
- Conduct initial evaluations and ongoing assessments to determine PT needs.
- Develop, implement, and adjust individualized plans of care focused on functional goals.
- Provide therapeutic interventions including therapeutic exercise, gait training, and use of modalities as appropriate.
- Document evaluations, progress notes, and discharge planning in a timely, compliant manner.
- Collaborate with an interdisciplinary team to optimize patient outcomes and safety.
- Educate patients and families on home exercise programs, precautions, and expectations.

Requirements
- Active New York state license to practice physical therapy.
- BLS (Basic Life Support) certification.
- Prior experience in a Skilled Nursing Facility or SNF setting required.
- Ability to work Monday–Friday, 8:00 am–4:30 pm.
- Strong communication and documentation skills.
- Eligible to work in the listed travel assignment setting.
