Full Time Physical Therapist - Home Health - Up to $67/unit

Full Time Home Health Physical Therapist

Provide patient-centered care in patients' homes, performing evaluations, developing treatment plans, and documenting progress in Matrix EMR. Works Monday-Friday with supportive team and flexible autonomy.

Key Responsibilities:

1. Perform comprehensive patient evaluations and develop individualized treatment plans
2. Conduct start of care assessments and complete OASIS documentation accurately
3. Provide routine home health therapy visits focused on improving strength, balance, mobility, and independence
4. Complete patient recertifications and reassessments as required
5. Educate patients and caregivers on therapeutic exercises, mobility strategies, and home safety
6. Collaborate with physicians, nurses, case managers, and interdisciplinary healthcare teams
7. Monitor patient progress and modify treatment plans to maximize outcomes
8. Maintain timely and accurate clinical documentation using Matrix EMR
9. Deliver compassionate, patient-centered care in a home-based setting

Qualifications:

• Doctorate or Master's degree in Physical Therapy from an accredited program
• Active Missouri Physical Therapist (PT) license required
• Strong communication, organization, and time management skills
• Ability to work independently and manage a daily patient schedule
• Experience with OASIS documentation and Matrix EMR preferred

Preferred Skills:

• Current CPR/BLS certification
• Experience with OASIS documentation and Matrix EMR
