An excellent opportunity is available for a skilled and compassionate Home Health Registered Nurse (RN) to deliver patient-centered care in a field-based environment. This 13-week contract role focuses on providing comprehensive nursing care to patients in their homes, requiring a high level of independence, organization, and clinical expertise.

Schedule/Hours:

• Full-time, 13-week contract
• Monday Friday, 8:00 AM 5:00 PM
• Caseload: 2532 points per week (typically 35 patient visits daily)

Key Responsibilities:

• Conduct comprehensive OASIS assessments and devise individualized care plans
• Deliver skilled nursing: wound care, medication administration, IV therapy, and chronic disease management
• Evaluate patient progress and quickly communicate status changes to interdisciplinary team and physicians
• Educate patients and families regarding diagnoses, treatment, medications, and preventative care
• Document care accurately and promptly using Home Care Home Base (HCHB)
• Collaborate with physical therapists, occupational therapists, social workers, and home health aides
• Attend case conferences, staff meetings, and continuing education as required
• Adhere to regulatory and agency standards for care and documentation

Qualifications:

• Active RN license (state of practice, unencumbered)
• At least 1 year of recent home health field experience
• Proficiency with Home Care Home Base (HCHB) and OASIS required
• Strong assessment, organizational, and time management skills
• Reliable transportation, valid drivers license, and proof of auto insurance
• Ability to work independently and efficiently in a field-based position
• BSN preferred
• Prior experience with caseloads of 2532 points per week a plus
• Understanding of Medicare/home health compliance standards beneficial

$2,069 - $2,189/Week Estimated weekly earnings represent hourly wages, based on 40 hours per week, and expense reimbursement for eligible candidates. The actual amount may vary based on hours worked, eligibility for reimbursements, and seasonal adjustments.
