Care Manager Registered Nurse

Role Overview: This position involves providing case management services for inpatient and observation patients, focusing on care transitions, discharge planning, and risk assessment. The role requires collaboration with healthcare teams, social services, and outside agencies to ensure appropriate post-acute care and resource utilization. Key Responsibilities: Conduct comprehensive assessments of patients' medical, psychosocial, and discharge needs; develop and coordinate individualized care plans; evaluate patient progression using evidence-based tools like InterQual; act as a liaison among physicians, patients, families, and staff; and elevate care transition issues as needed. Qualifications & Skills: Minimum of 2+ years in case management or 3+ years in a clinical setting; Associates Degree in Nursing (preferably a Master's); current RN license; BLS certification; and experience with InterQual is preferred.
