UT Health ET Home Health Patient Service Coordinator "LVN" Full-Time TX - Tyler Home Health Branch, Tyler, TX, US UT Health East Texas Home Health is seeking a Licensed Vocational Nurse (LVN) to serve as an Office Patient Service Coordinator. This role is the central hub of our home health office—supporting daily operations while ensuring a professional, welcoming experience for patients, families, staff, and referral partners. This is an ideal opportunity for an LVN who enjoys both clinical coordination and administrative responsibilities in a fast-paced healthcare environment. Key Responsibilities Serve as the front office point of contact; greet visitors and manage office flow Answer and route incoming calls in a timely, professional manner Provide administrative and clerical support to clinical and leadership teams Assist with patient coordination, communication, and documentation support Perform data entry and maintain accurate records and office directories Coordinate office supplies, vendors, mail, and deliveries Support meeting coordination, travel arrangements, and team events Uphold patient confidentiality and ensure HIPAA compliance Communicate effectively with patients, families, clinicians, and referral sources Represent the organization with professionalism and a service-focused approach Qualifications Current LVN license in the state of Texas (required) High School Diploma or equivalent required Minimum of 1 year of administrative or healthcare office experience preferred Strong organizational and multitasking skills Excellent communication and customer service abilities Proficiency in Microsoft Office and data entry systems Knowledge of medical terminology and home health workflows preferred Additional Requirements Reliable transportation, valid driver’s license, and current auto insurance Ability to work a flexible schedule, including occasional extended hours Office-based role within a home health setting (not a remote position) Fast-paced environment with frequent interaction across teams #J-18808-Ljbffr
