Details Home health setting covering Area #1 (Fremont, Castro Valley, Ashland, Cherryland, Hayward, Newark, San Leandro, San Lorenzo, Union City) and Area #2 (Oakland, Berkeley, Alameda, Albany, Emeryville, Piedmont) Day shift schedule 13-week assignment length Weekend coverage may be required Travel outside the assigned area may be required to maintain caseload Requirements Current CA PT license required if applicable BLS certification required Minimum 1 year of experience required Valid driver's license, auto insurance, and auto registration required Must provide documentation of TB test (annual), MMR, Varicella, Hepatitis B, Tdap (within 10 years), and annual flu vaccination Must pass a drug screen and physical exam Must be willing to see COVID-positive and COVID PUI patients Additional Information Responsibilities include performing home visits such as evaluations, re-evaluations, discharges, and OASIS assessments Caseload includes a mix of routine visits, evaluations, and OASIS-related visits Compensation for visits is calculated using a point-based system that converts to approved hours Former employees are not eligible for this travel assignment All requested time off must be disclosed prior to assignment confirmation; time off requested after acceptance will not be approved American Traveler Job ID #P-758460.
