About the Role: Provide direct patient care to residents, including administering medications, wound care, and implementing treatment plans. Conduct routine vital sign checks, report changes in condition, and collaborate with the interdisciplinary team to develop and update care plans. Develop and maintain positive relationships with residents, families, and colleagues to ensure a smooth transition from hospital to home. Participate in quality improvement initiatives to improve patient outcomes and satisfaction. Responsibilities: Provide skilled nursing care to residents with complex medical and psychosocial needs. Collaborate with other disciplines to develop and implement individualized care plans. Conduct condition-specific education and training for residents and families. Participate in complex wound care, ostomy care, and other special care programs. Maintain accurate and detailed records of care provided and resident outcomes.
