Ambulatory Care Pharmacist
The Ambulatory Care Pharmacist is expected to be an integral part of an outpatient, multidisciplinary healthcare team to improve patient outcomes with a focus on medication management by overcoming any access barriers and providing necessary guideline directed therapy. This role will be responsible for ensuring integrated, accessible health care services. The Ambulatory Care Pharmacist will be accountable for addressing medication needs, developing and sustaining partnerships with patients and other healthcare providers, and practicing in the context of what is best for our patients and community.

Responsibilities:
- Responsible for properly assessing patients' pharmaceutical needs and providing pharmaceutical care.
- Responsible for addressing any medication access issues and providing care coordination if needed.
- Responsible for building and maintaining partnerships with patients and other healthcare providers to help build a strong multidisciplinary healthcare team that works to improve patient outcomes.
- Responsible for staying up-to-date on therapy guidelines and advancement in Ambulatory Care Pharmacy.
- Responsible for properly counseling patients/patients' families, and appropriate health care professionals regarding drug therapy.
- Responsible for ordering, interpreting, and/or monitoring laboratory testing if necessary.
- Responsible for monitoring medication therapy adherence.
Activities:
- Performs medication reconciliations with patients to not only obtain accurate medication lists, but also to identify any pharmaceutical needs that are not being met.
- Monitors patient's drug therapy to detect/prevent drug interactions, contraindications, and other adverse reactions.
- Makes appropriate interventions to improve patient care that is based on current guideline-directed therapy.
- Performs preventative care (ex. screening and administering vaccinations) or wellness screenings.
- Uses, is proficient in the operation, and documents within the pharmacy computer system and the electronic medical record.
- Obtains pertinent patient medication/medical information.
- Provides current and accurate drug information to patients and health care professionals.
- Communicates effectively with patients, health care professionals, and coworkers.
- Dispense and/or administer medications to patients in accordance with ARH policies, including but not limited to, all routes of immunizations, naloxone, and injectable medications where permitted by law.
- Complete and properly document medication therapy management interventions.
- Ensures compliance with company and legal policies, procedures, and regulations.
- Ensures confidentiality of information, documentation, and records.
- Supports the pharmacy manager or specialty pharmacist as needed.
- Perform other related duties as assigned.
Qualifications
Education: Bachelor of Science degree in Pharmacy or Pharm D. Current state pharmacist licensure in good standing or licensure eligibility in state of employment within 90 days of hire. Immunization certification.
Minimum Work Experience: Residency training (PGY-1) preferred or previous Ambulatory Care experience or 1-2 years of retail experience.
Required Skills, Knowledge, and Abilities: Be a cooperative team member with strong written and oral communication skills. Ability to collaborate and build relationships with other healthcare professionals and patients. Motivation to stay up-to-date on therapy guidelines and advancement in Ambulatory Care Pharmacy. Board certification, clinical experience, and continuing-education certification programs are encouraged in Ambulatory Care Pharmacists participating in direct patient care.
