h3>Overview/h3>p>#LI-RT1/p>p>b>This role is based in an Assisted Living, Independent Living, and Memory Care setting./b/p>p>b>PRN/b/p>p>b>Summary:/b/p>p>Provide Speech‑Language Pathology Therapy and related services for patients while maintaining positive levels of interaction with facilities and clients in accordance with the principles and practices of Speech‑Language Pathology therapy and within Reliant Rehabilitation guidelines. Provide quality patient care including screening and evaluation of patients, development of initial and ongoing treatment plans of care, discharge plan, supervision of assistants and Rehab Techs, and case management responsibilities. Maintain professional conduct that represents Reliant Rehabilitation’s commitment to quality and service in all interactions with clients, including positive levels of interaction and rapport with facilities and clients. Adhere to all state and federal regulations regarding speech‑language pathology therapy./p>h3>Responsibilities/h3>ul>li>Optimize communication, cognitive‑linguistic abilities, and swallowing skills of patients by providing, directing, and/or overseeing speech therapy in the following areas:/ul>ul>li>Screening of residents, as appropriate;/li>li>Evaluating and treating patients who may benefit from therapy as referred by state‑approved referral sources;/li>li>Complying with state and federal guidelines regarding accurate and timely billing and documentation in the medical record;/li>li>Establishing effective interactions with patients, interdisciplinary team, and other clients which reflect patient‑centered care./li>li>Directly supervising Rehab Techs during group and individual treatments and building a strong Speech‑Language Pathology (SLP) therapy program, including:/ul>ul>li>Active integration of the SLP therapy’s role into community and interdisciplinary treatment teams;/li>li>Contributing to community patient care, utilization review, case management, administrative staff/department head, and family conference activities and communications;/li>li>Establishing and maintaining a good working relationship with other health care professionals and colleagues in the community;/li>li>Making appropriate referrals to other members of the rehabilitation team;/li>li>Developing and implementing new programs/techniques to enhance rehabilitation services;/li>li>Adhering to Reliant Rehabilitation and client‑specific policies and procedures related to environmental safety, orderliness, risk management, infection control, and emergency preparedness;/li>li>Adhering to the requirements of patient rights and privacy./li>li>Enhancing clinical expertise, professional and management skills through interaction with managers, therapists and other professional staff, self‑study, and other continuing education activities./li>li>Maintaining valid state license (where appropriate) and staying aware of issues related to the profession of speech‑language pathology and the health care environment./li>li>Understands various relevant payment models related to billing and treatment guidelines and delivering quality care within guidelines./li>li>Complying with infection control procedures and environmental safety protocols within a community./li>li>Performing patient lifting using appropriate biomechanical techniques – capable of lifting up to thirty (30) pounds of dead weight alone./li>li>Maintaining confidentiality of Reliant Rehabilitation, community, and patient information./li>li>Conducting self‑care in an ethical, legal, and responsible manner at all times./li>li>Adhering to the policies, principles, and guidance within the Team Member Handbook and Code of Business Conduct./li>li>Traveling between facilities during scheduled hours./li>li>Working varying hours and shifts, including weekends and holidays./li>li>Attending all mandatory meetings, training, and assignments as delegated./li>li>Performing other duties and responsibilities as assigned./li>h3>Qualifications/h3>ul>li>b>Competency:/b> Prior to beginning patient care for Reliant Rehabilitation, a peer SLP may complete a Competency Checklist. The following competencies are required:/ul>ul>li>Effective clinical problem‑solving skills, including the ability to identify, prioritize, develop action plans, and execute them in a timely manner;/li>li>Strong teamwork skills;/li>li>Strong verbal and written communication skills;/li>li>Ability to build strong relationships with a diverse population;/li>li>Proficiency in reading, writing, speaking, and comprehending in English for instructions, correspondence, charts, memos, and reports;/li>li>Ability to work in a fast‑paced, productive work environment./li>li>b>Education/Experience:/b> Master’s Degree, or equivalent, in Speech Language Pathology./li>li>b>Computer Skills:/b> Proficient knowledge of Microsoft Office (Outlook, Word, Excel, Power Point, etc.) and basic Internet navigation skills./li>li>b>Certificates and Licenses:/b> Valid Speech‑Language Pathologist state license (where appropriate). #J-18808-Ljbffr Reliant Rehabilitation
