This is an OPEN CONTINUOUS ANNOUNCEMENT and will remain open until Sept 30, 2026. The initial cut-off date for referral of eligible applications will be July 27, 2026. Eligible applications received after that date will be referred at regular intervals on an as-needed basis until positions are filled. All referred applicants receive a final notification once a selection decision has been made.

Major duties include but are not limited to:

• Coordinate timely, Veteran-centered care for Veterans receiving services through VA and community care pathways.
• Triage requests for services and help direct Veterans to appropriate care options based on clinical needs and established processes.
• Develop and support care coordination plans that promote continuity, access, and successful completion of the episode of care.
• Review medical documentation from community providers to support informed care coordination and follow-up.
• Serve as a liaison between Veterans, VA care teams, and community providers throughout the consult and referral process.
• Guide Veterans through available care options, including direct VA care and care in the community.
• Collaborate with interdisciplinary teams to support timely referrals, scheduling, communication, and care outcomes.
• Document, exchange, and track care coordination information using VA health information systems and required care coordination processes.

Pay: Competitive salary

Licensure: 1 full and unrestricted license from any US State or territory

Work Schedule: This is an intermittent position. Intermittent employment is defined as non-fulltime employment in which employees are appointed without a regularly scheduled tour of duty. Work is sporadic and unpredictable so that a tour of duty may not be regularly scheduled in advance.
