Family Medicine Physician – Hybrid Primary Care (Home Visits + Clinic) | Brooklyn, NY

Community-based primary care practice in Brooklyn is seeking a full-time Family Medicine Physician to provide a hybrid model of outpatient care in Bedford-Stuyvesant, combining in-office visits with scheduled home care.

This opportunity is ideal for physicians who value continuity of care and hands-on community medicine.

Position Overview

The physician will provide comprehensive primary care to adult patients in a hybrid format, typically split between 2–3 days of home visits and 2–3 days in-office. Responsibilities include preventative care, chronic disease management, wellness visits, and ongoing patient follow-up.

This is a fully outpatient, permanent role supported by an established patient base and experienced administrative staff.

Qualifications

• MD or DO

• Board Certified or Board Eligible in Family Medicine

• Active New York license (or eligibility)

• Outpatient primary care experience preferred

• Comfortable with hybrid care model

• EMR proficiency
