LICSW-Case Manager, Social Work- In Patient III Washington, DC Capitol Hill Med. Ctr. (40hrs.)

Job Summary This role is eligible for a $10,000 sign‑on bonus! The position is responsible for collaborating with the health care team to facilitate the coordination of appropriate, cost‑effective services for a defined population, achieving members’ optimal level of independence and enhancing quality of life. Essential Responsibilities Problem identification, psychosocial assessment, financial counseling/referral, accessing community resources, and placement for care. Guiding members through health‑related legal processes or consulting and supporting other health care professionals. Effectively managing and coordinating an assigned caseload in compliance with established criteria. Completing comprehensive psychosocial assessments to evaluate goals, social support, resources, health status, functional limitations, psychological state, environmental factors, and treatment response. Collaborating with the nurse case manager and other team members to develop and monitor a plan of care that promotes optimal functioning and quality of life. Identifying, facilitating, and advocating for appropriate organizational and community resources, ensuring implementation in a cost‑effective, efficient, and timely manner. Maintaining consistent and reliable documentation of case‑management activities in compliance with all organizational and departmental standards. Analyzing patient and program outcomes to identify improvements in program, quality, and cost effectiveness of case‑management activities. Facilitating application processes for local, state, and federally funded programs (e.g., Medicaid, Medicare, Disability) and/or referring to appropriate community agencies when abuse/neglect is suspected. Providing supportive counseling and education to members, families, caregivers, and the community, including end‑of‑life issues and advanced directives. Promoting self‑awareness and knowledge of current case‑management standards and recent innovations in patient care. Scheduling and coordinating family meetings as needed. Completing guardianship paperwork and providing technology assistance so that patients/families can attend court proceedings virtually, when necessary. INPATIENT ONLY – Completion of Uniform Assessment Instruments (UAIS) form for long‑term care (Virginia Medicaid requirement). Performing other related duties as assigned. Basic Qualifications Minimum one (1) year of clinical social work experience in a health care setting. Master’s degree in social work (MSW). Credentials from multiple states: primary state credential required at hire; secondary state credentials required post‑hire. Independent Clinical Social Worker License (District of Columbia) within 6 months of hire. Licensed Certified Social Worker (Maryland) within 6 months of hire. Licensed Clinical Social Worker (Virginia) within 6 months of hire. Experience with Windows-based computer software programs. Knowledge of community systems, resources, and regulatory issues for the Mid‑Atlantic area. Preferred Qualifications Experience with computer software programs in a Windows environment. Knowledge of community systems and resources in the defined service area. Knowledge of regulatory issues for the Mid‑Atlantic area. Employment Details Primary Location: District of Columbia, Washington, Capitol Hill Medical Center. Full‑time, 40 hours per week (evening shift). 3:00 PM – 11:30 PM. Travel required (approximately 20% of the time). Worker location must align with Kaiser Permanente’s Authorized States policy.
