Location: Jackson Heights, NY (Onsite)

Employment Type: Full Time - Mon-Fri 09:00am-05:30pm

Compensation: Pay Range: $68,140.80-$71,198.40 Per Year

Description

This position involves providing behavioral health case management services in a primary care clinic setting. The role is full-time and located onsite in Jackson Heights, NY, with standard weekday hours. The successful candidate will use various technology platforms to manage patient records and coordinate care.

Requirements

Advanced degree in mental health counseling or social work. Two years of work experience in a related field. Licensure with LMHC or LMSW required. UAS-NY certification within one month of hire. Experience working with diverse populations. Excellent written and verbal communication skills in English. Ability to work flexible hours including some evenings and weekends.

Qualifications

Proven experience in behavioral health case management. Strong crisis intervention and counseling skills. Ability to coordinate and link patients with additional services. Experience in maintaining accurate patient records. Proficiency in using various technology applications. Fluency in Spanish or an Asian language is a plus. Excellent customer service skills.
