Opportunity Details: Able to work evenings and weekends. Collaborates to provide care coordination activities in providing comprehensive care (e.g., other healthcare providers, local health departments, social service, nutritionist, dentist, community-based organizations, home care and patient education groups). Provide relevant health education and instructions, counsels patients/family responsible on the impact of illness and principles of health promotion and maintenance. Provides clinical and didactic instruction to professional staff, peers, students and community groups and conducts in-service educational programs, as needed. Participates in departmental meetings and interdisciplinary community committees, as well as peer review performance improvement meeting activities. Peer review. Refer to specialty services. Qualifications Board Eligible or Board Certified by the NCCPA Current NYS registration as a Physician Assistant Wages and Benefits include: Annual Base Salary: $131,127* - $174,836* based on 40-hour work week. The annual total value of compensation package is estimated at $144,239** - $192,319**, which includes the baseline salary, 401(k) contribution and other factors as set forth below: 401(k) Company Contribution (subject to IRS contribution limits): Employees are immediately vested in a 3% company contribution of base earnings. No employee match is required. After one year of service, employees receive an additional 7% company contribution of base earnings. No employee match is required. Annual Continuing Medical Education (CME) Reimbursement. Generous Annual Paid Time Off (PTO): Vacation, Sick, Holiday, and CME days.
