Hospice Admission RN — Compassionate Home Care
Wellstar Health System
Wellstar Health System seeks an experienced Hospice Admission RN to educate patients and families about hospice services and the palliative approach. You will perform admissions for home, ALF, or SNF settings, conduct nursing assessments, determine eligibility, and collaborate with the hospice team to develop the initial plan of care. You will collect orders, obtain verbal certification, and coordinate with the attending physician and the Medical Director. #J-18808-Ljbffr Wellstar Health System
