About the Position

Job Title: Physical Therapist - Home Health Profession: Physical Therapist Specialty: Home Health Duration: 13 weeks Shift: Day Shift Hours per Shift: Must have EPIC experience Experience: Doctor of Physical Therapy (DPT) degree required; only if obtained license on or after January 2016, or foreign equivalent License: Physical Therapy licensure in all applicable states of practice required Certifications: Cardiopulmonary Resuscitation (CPR) certification or Basic Life Support (BLS) certification from approved vendor required Must-Have: Valid Drivers License and must be at least 21 years of age or older Description: Our home health agencies serve the entire community and offer personalized in-home care. We plan and conduct segments of treatment programs designed to restore and improve physical, social, and mental functions, while meeting department objectives. We serve as co-leaders in an interdisciplinary approach to patient care. We complete evaluations to ensure quality standards and address assessment areas that require attention. We use appropriate clinical techniques and tools and establish individualized treatment plans for patients. We provide therapy interventions using appropriate treatment protocols and techniques. We establish individualized care and discharge plans and ensure they are communicated to appropriate parties. We complete comprehensive and accurate clinical documentation within established deadlines. We supervise assigned assistants. We perform other job-related duties as assigned.
