Physical Therapist - Travel Contract

Minimum Requirements

• Bachelor's, Master's, or Doctoral degree in Physical Therapy from an accredited program.
• Active California PT license required to start the assignment.
• BLS certification may be required from AHA or ARC.
• Minimum 1 year of home health experience at a Medicare-certified agency. (Client-specific requirement.)
• Experience with HCHB electronic medical record required. (Client-specific requirement.)
• Productivity familiarity with treating approximately 6–7 patients per day preferred.

Assignment Details

• Facility Type: Home Health.
• Shift Name: Days.
• Shift Details: Monday–Friday, 8:00 AM–5:00 PM.
• Schedule: Full-time, 40 hours per week.
• On-call or weekend requirements: No.
• Territory: Napa County, North. Candidate may be expected to travel within the assigned county.
• Duration: 13 weeks.
