Community Health Navigator & Outreach Advocate

The City and County of San Francisco is seeking a Health Worker 2 to perform paraprofessional duties in the Department of Public Health. Responsibilities include serving as a liaison between community residents and program staff, providing counseling to patients, and assisting them in utilizing healthcare services.
