On-Site LPN Clinician: Health Navigator & Coach
Hamilton Health Box, Inc.
Hamilton Health Box (HHB) is hiring an On-Site Clinician (OSC) to lead daily clinical and administrative operations at HHB locations. You will interact with employees for scheduling and follow up, deliver protocol-driven care, and implement preventative health initiatives. The OSC coordinates care with providers and partners, supports health education, and builds strong relationships with staff, patients, and the community.
