Physician Assistant - Geriatrics

SUMMARYAs a Physician Assistant in Geriatrics, you’ll provide comprehensive primary and acute care to older adults across both Long-Term Care and Assisted Living settings. This role offers the opportunity to build meaningful relationships with patients while managing a diverse range of acute and chronic conditions. Provide primary and acute care for geriatric patients, including assessment and management of acute and chronic conditions, physical examinations, diagnosis, treatment planning, medication management, and follow-up careLong-Term Care. Provide care 2–3 days per week at Unity Living Center, including management of long-term care residents and patients requiring long-term ventilator support in collaboration with respiratory therapyAssisted Living. Provide primary care 2–3 days per week to residents in assisted living settings, working closely with facility staff and the Geriatric Medical DirectorCare Coordination. Collaborate with nursing, respiratory therapy, facility staff, physicians, and other members of the care team to coordinate comprehensive patient careDocumentation. Maintain accurate medical records, complete required documentation, and monitor patient response to treatmentEDUCATION:BS: Physician Assistant (Required)LICENSES / CERTIFICATIONS:PA-C - Certified Physician Assistant - National Committee on Certification of Physician Assistants (NCCPA), RPA - Registered Physician Assistant - New York State Education Department (NYSED)PHYSICAL REQUIREMENTS:M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.For disease specific care programs refer to the program specific requirements of the department for further specifications on experience and educational expectations, including continuing education requirements.Any physical requirements reported by a prospective employee and/or employee’s physician or delegate will be considered for accommodations.PAY RANGE:$100,000.00 - $150,000.00CITY:RochesterPOSTAL CODE:14626The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant.
