Primary Care Physician – Conviva Senior Primary Care

Location: Deerfield Beach, FL.

Summary

The Primary Care Physician (PCP) leads a team to provide value‑based care focused on quality for seniors. The PCP collaborates with doctors, advanced practice professionals, pharmacists, nurses, behavioral health, specialists, coders, and referral coordinators to deliver a comprehensive patient experience.

Key Responsibilities

• Evaluate and treat patients in accordance with standards of care.
• Follow level‑of‑care and quality protocols, monitor care using data and chart reviews.
• Coordinate patient services, including specialty referrals, hospital and SNF coordination, durable medical equipment, and home health care.
• Act as an active participant and key source of medical expertise with the care team through daily huddles.
• Assist Regional Medical Director and Center Administrator in setting a tone of cooperation by displaying a professional and approachable demeanor.
• Complete all medical‑record documentation timely, working with a quality‑based coder to optimize coding specificity.
• Follow policy and protocol defined by Clinical Leadership.
• Meet with RMD to review quality of care, outcome data, policy, procedure and records issues.
• Participate in potential growth opportunities for new or existing services within the Center.
• Participate in the local primary care “on‑call” program as needed.
• Ensure personal compliance with licensing, certification, and accrediting bodies.
• Allocate 100% of time to direct patient care, including charting and meetings related to patient care.

Required Qualifications

• Current and unrestricted medical license (or willing to obtain one) in the state of practice; eligible to obtain licenses in other states if required.
• Graduate of an accredited MD or DO program.
• Excellent verbal and written communication skills.
• High level of interpersonal skills with colleagues and patients.
• Fully engaged in the “Integrated team‑based care” model.
• Willingness and ability to learn and adapt in a value‑based care setting.
• Superior patient/customer service orientation.
• Basic computer skills, including email and EMR.
• Required to participate in the Tuberculosis screening program and be screened when selected.

Preferred Qualifications

• Board certification or eligibility for certification (ABMS or AOA) in Family Medicine, Internal Medicine or Geriatric Medicine.
• Active and unrestricted DEA license.
• Medicare Provider Number.
• Medicaid Provider Number.
• Minimum of two to five years of directly applicable experience.
• Experience managing a Medicare Advantage panel with an understanding of best practice in coordinated care environments.
• Knowledge of Medicare guidelines and coverage.
• Bilingual ability (plus).
• Knowledge of HEDIS quality indicators.

Compensation & Benefits

Scheduled Weekly Hours : 40

Pay Range : $203,400 – $299,500 per year (starting base pay; varies by location and qualifications). Eligible for a bonus incentive plan.

Benefits include a guaranteed base salary with quarterly bonus, health insurance effective on the first day, CME allowance/time, occurrence‑based malpractice insurance, 401(k) with employer match, life and disability insurance, paid time off and holidays, and minimal call.
