Job Title: Home Health RN Profession: RN Home Health Specialty: RN Home Health Duration: 13 weeks Shift: 08:00 AM - 04:30 PM Hours per Shift: 8 Experience: Minimum of 1 year License: Active RN license Certifications: None specified Must-Have: 1 year Certified Home Care experience, 1 year Case Management experience, ability to travel to cases as assigned. Description: As a Home Health RN, you will be responsible for assessing patient conditions. You will implement care plans and provide ongoing education and support to patients and their families. Patient care is delivered within the organizational philosophy and policies. You will also perform admission processes and case management responsibilities according to established policies and standards of practice. Utilizing thorough and timely electronic documentation and interdisciplinary communications is required. Weekend rotations are once every 6 weeks. Start date is based on credentialing clearance. If not cleared by the posted start date, a rescheduled start date will be necessary.
