(depending on experience) Job Summary: We are seeking a skilled Physical Therapist (PT) to join our team in providing exceptional rehabilitation services to residents. As a PT, you will assess, plan, and implement personalized therapy programs to enhance mobility, strength, and overall quality of life for patients in our skilled nursing and rehab facility. Conduct assessments to determine physical therapy needs and goals. Develop and implement individualized treatment plans for residents. Provide hands‑on therapy to improve strength, balance, and mobility. Collaborate with the healthcare team to optimize patient care. Document progress and modify treatment plans as needed. Educate residents and their families on techniques for improving daily function. Adhere to infection control practices and regulatory standards. Required Skills: Current Physical Therapist license in New York State (NYS). Strong assessment and therapeutic skills. Ability to work effectively in a multidisciplinary team. Excellent communication and interpersonal skills. Passion for patient care and improving quality of life. Pay Pay or shift range: $34 USD to $37.73 USD. The estimated range is the budgeted amount for this position.
