Job Details:

Setting: Home Health

Pay Range: 1861.48 - 1954.55

Schedule: Mon - Fri 8am - 5pm with on call and weekends (On Call Schedule: at least 1 weekend per month)

Contract Length: 13 weeks

Requirements: Home Health Experience, HCHB, OASIS

Responsibilities:

• Assess, plan, implement, and evaluate nursing support for individuals in a community setting
• Coordinate communication among various health disciplines to support health and safety
• Review and monitor medications as prescribed, ensuring proper policies are in place
• Complete and maintain thorough documentation of health status and care activities
• Supervise licensed practical/vocational nurses and ensure timely health appointments and responses
• Provide training for new employees and ensure unlicensed staff are trained on person-specific health needs
