Registered Nurse - Case Manager

Job Highlights Location: WI-SSM Health St. Mary's Hospital – Madison Schedule: Monday – Friday 8:00am – 4:30pm with every 12th‑weekend rotation and one required holiday per year Prior case management and/or Med/Surg or Post‑Acute hospital experience preferred Job Summary Assesses, develops, plans, implements, and evaluates patients’ discharge planning needs to the next level of care in collaboration with the interdisciplinary team. Responsibilities Ensure compliance with Condition of Participation (CoP) discharge planning. Identify and escalate gaps/barriers related to inpatient and observation length of stay. Assess patients at high risk for readmission and identify root causes. Develop plans and collaborate with multidisciplinary team regarding discharge planning to next level of care. Participate with other team members regarding opportunities for improvement in standard work. May be responsible for utilization management process, ED case management process, admission process, and act as a case manager liaison with post‑acute providers (skilled nursing facilities, rehab, home health, etc.). Apply evidence‑based practice and scientific knowledge in health care to nursing practice, ensuring patient care is based on age‑specific and clinical needs. Demonstrate professional nursing standards as defined by the professional practice model. Use the ANA Code of Ethics for Nurses to guide response to evolving health and nursing needs. Work in a constant state of alertness and safe manner. Perform other duties as assigned. Education & Requirements Graduate of an accredited school of nursing or education equivalency for licensing. Three years’ acute care experience. Physical Requirements Frequent lifting/carrying and pushing/pulling objects weighing 0‑25lbs.; occasional lifting of 25‑50lbs. Frequent sitting, standing, walking, reaching, and repetitive foot/leg and hand/arm movements. Frequent use of vision and depth perception for distances near (20inches or less) and far (20feet or more) and to identify and distinguish colors. Frequent use of hearing and speech to share information through oral communication; ability to hear alarms and machinery. Frequent keyboard use/data entry. Occasional bending, stooping, kneeling, squatting, twisting, and gripping. Occasional climbing.
