Cleveland Clinic - Mellen Center

Cleveland Clinic Fairview Hospital is seeking an RN Care Coordinator for Medical Oncology. The role involves coordinating care for high-risk oncology patients, collaborating with a multidisciplinary team, and focusing on patient education and transition of care. The position supports reducing readmissions and improving patient outcomes. The caregiver will work 8:00am-4:30pm, with no weekends/holidays/on-call required, at Fairview Hospital's Cancer Center Physician Suite. #J-18808-Ljbffr Cleveland Clinic
