Highland Rivers provides treatment and recovery services to individuals impacted by mental illness; substance use disorders and intellectual developmental disabilities. Job Summary The Substance Use Clubhouses at Highland Rivers Behavioral Health provides a fun and safe environment for youth ages 13 to 18 to hang out, socialize and learn without pressure or the temptations of substance use, and provides comprehensive services to engage youth as well as their families in their recovery. Job Duties and Responsibilities Assist youth with service needs, developing strategies and supportive, social support networks and coordinating rehabilitation services as specified in the consumer’s individual recovery plan. Partner with the individual and CORE services provider in the development of the individual service plan. Assist the individual with the transitioning between levels of care. Link and serve as advocate with other agencies to garner needed resources for consumers. Provides assistance and support in crisis situations. Provides assessment, monitoring, and assistance to individuals. Establishes professional and ethical relationships with youth, families, and coworkers Provides direct service to individuals; completes all necessary paperwork according to policy; ensures individual meet eligibility criteria or refers to appropriate provider in the community. Manages a caseload by ensuring all services are authorized, chart paperwork is complete and current, and consumer charts are closed according to policy. Sees a caseload at the frequency defined in the service plan; documents attempt to contact individuals and ISP reviews; completes monthly caseload reports; track appointments and case management. Other duties as assigned. Must be able to pass background investigation, drug screen and have good 7year motor vehicle report. Bilingual abilities a plus.
