Nurse Practitioner

Contract Length: 6+ months Location: Mishawaka, IN 46544 Shift: FT, 30-35 hours weekly, every other weekend. Hours for clinic are Mon-Fri 8a-7p (1 hour lunch) Sat 9a-530p (30min lunch) Sun 9a-430p (30min lunch) Candidates must have prior primary care or urgent care experience. Do not submit candidates who do not meet this requirement.

Primary Responsibilities

• Client Nurse Practitioners will work in collaboration with a dedicated clinical care team to provide evidenced-based care to a panel of patients.
• The clinical care team will work with the provider daily to drive better outcomes for entire panel of patients.
• This will include accurate assessment, diagnosis, treatment, management of health problems, health counseling, and disposition planning.
• This role will report to the practice manager.
• Include screenshot of BLS and license. (BLS from ARC or AHA is required.)

Experience:

• A MINIMUM of 1 year of recent Primary Care experience as a Nurse Practitioner in either a family medicine, primary care practice, or urgent care required.
• Working knowledge and understanding of quality measurement in the management of chronic disease conditions
• Self-motivated, prioritizes and solves problems, takes initiative, and advocates for their patients and their practice

Education:

• Master's Degree level Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role.
